Steps:

These steps are for the webmaster, please remove this section after you are done with the form customization.

  1. Modify the form body to meet your needs where you can add or remove form elements.
  2. Modify the JavaScript code if you want to process the data before it is submitted.
  3. After building the form, you will need to submit a request to get new Form ID which will allow your form to submit its data. To request a new Form ID, click here.
  4. When you receive the new Form ID, modify the form action and insert the new Form ID.
  5. Now your form is ready to collect and submit information.
